Jail for third man involved in death of crew member who was given unsafe breathing mask

Wed May 20 10:27:50 CEST 2026 Timsen

On May 19, 2026, Tien Manh , the once chief officer of the 'GT Win', 32, was sentenced to two years and nine months’ jail after he pleaded guilty to committing a rash act linked to the death of the Vietnamese crew member Hoang Van Chau, 40. He had fallen unconscious in a tank that had been used to transport naphtha gas, and died from exposure to volatile hydrocarbons. Manh also admitted to one count of giving false information to a public servant. The tanker was berthed in Singapore, told four men to clean tanks that had not been checked and declared gas-free, resulting in the death of one of them from exposure to volatile hydrocarbons. He had also directed and allowed the use of a modified self-contained breathing apparatus (SCBA) for the task despite knowing that this was dangerous. Following the death, he lied to the authorities on two occasions, resulting in the omission of investigations. In February, Nguyen Duc Nghi, then 50, who was the captain of the 'GT Win' at the time of the incident, was sentenced to one year and two months’ jail for performing an act that could obstruct the course of justice. He had told his crew to lie to the authorities about the circumstances leading to Mr Chau’s death. Another man, Le Thanh Dung, then 36, who was a pump master on the tanker, was sentenced to three months and two weeks’ jail in July 2025. He had modified two SCBA masks despite knowing that they were unsafe to use when cleaning the tanks. The 'GT Win' had sailed from Thailand to Singapore in May 2024 with tanks of naphtha gas as cargo. After the cargo was unloaded at a terminal on Jurong Island, the vessel anchored at the Raffles Reserved Anchorage. On May 14, Manh told Chau and three other crew members to clean some tanks that had not been declared gas-free. Manh’s role was to check for the oxygen level in the tanks before cleaning and perform the requisite safety checks before the tanks were cleaned by the crew. Manh also instructed Dung to modify an SCBA mask for the men to use. The modification involved attaching a connector of the mask to a hose linked to the valve of an air bottle on the deck. The hose was then secured with a clamp to prevent air leakage. The DPP said the modification was made as it was “impractical” for the men to don the heavy equipment while entering the tank owing to space constraints. It was not stated in court documents how an unmodified SCBA mask was supposed to function, or what the standard operating procedures were. At around 4.40 p.m., Manh instructed Chau to use the modified breathing apparatus to enter and clean a tank. hile in the tank, Mr Chau fell unconscious and was recovered. Chau was eventually taken to Marina South Pier, where he was pronounced dead at around 10.40 p.m. Dung showed Nghi the modified mask that Mr Chau had worn. Nghi flung the mask into the sea and told the crew members to lie to the police that the incident had taken place in another tank that was gas-free. Two days later, Manh lied to an investigation officer (IO), claiming that Mr Chau had entered the gas-free tank, and was later found unconscious inside it. Manh also told a similar lie to a captain from the Maritime and Port Authority of Singapore. Police officers confronted Manh on May 23, 2024, and he admitted that he had lied to the IO. As a result, the police were only able to collect gas samples from the correct tank after the accused’s confession, which was nine days after the incident. Manh was arrested in May 2025.

Captain sentenced to jail after lying about fatal incident

Wed Feb 11 10:45:18 CET 2026 Timsen

A court in Singapore has sentenced the Vietnamese captain Nguyen Duc Nghi, 50, of the 'GT Win' to 14 months in jail after he admitted to lying about a fatal incident aboard the vessel in 2024. was is the second officer from the ship, who was sentenced to jail for the incident that took the life of a seafarer ordered to clean a gas tank, while the case against Dao Tien Manh, the chief officer, was still pending in the courts. The gas tanker had arrived in Singapore on May 11, 2024, transporting a cargo of naphtha, offloaded its cargo and moved to the anchorage., where the chief officer ordered three crew members to clean the emptied tanks, failing to check the oxygen levels and make the necessary safety checks. Further, he instructed the pump master, Le Thanh Dung, to make modifications to the mask of the breathing apparatus. He connected the mask’s hose to an air bottle on deck, which prosecutors called “impractical", noting space constraints at a staircase. The victim, Hoang Van Chau, became unconscious while working in one of the tanks. The crew attempted CPR and requested emergency medical assistance from shore. Chau was pronounced dead at a local hospital, due to exposure to volatile hydrocarbons. The captainconcealed the facts of the incident from the Singapore Police Force and the Maritime Port Authority of Singapore. After the fatality, the captain spoke with the vessel’s pump master, who showed him the altered breathing mask. Due to the court filings, the master threw the mask overboard. Further, he instructed the crew that they should tell the investigators that Chau had been found unconscious in the tank and lie about the details. He said that was the only way the family would get the insurance money. Nghi was taken into custody on May 12. Crew members then told the investigators the truth about how the death happened. When the police confronted the captain, he admitted to have lied and instructed the crew to lie too. His actions were also equated to insurance fraud.

Pump master sentenced to jail for death of a crew member

Fri Jul 04 10:41:26 CEST 2025 Timsen

The Public Prosecutor in Singapore was proceeding with a case against the master, chief officer, and the pump master of the 'GT Win', fo the death of a crew member on May 11, 2024, after he entered a tank that had not been properly vented and inspected. The pump master pleaded guilty on July 2, 2025, to one count of acts endangering the life of the crew member. The chemical tanker had offloaded a cargo of naphtha and the moved to the Singapore anchorage, where the chief officer, Dao Tien Manh, ordered three crew members to clean the emptied tanks, although he failed to check the oxygen levels and did not carry out the necessary safety checks. Further, he instructed the pump master, Le Thanh Dung, to make modifications to the mask of the breathing apparatus. He connected the mask’s hose to an air bottle on deck, which prosecutors called impractical, noting space constraints at a staircase. One of the crew members, Hoang Van Chau, became unconscious while working in one of the tanks. Chau was found unconscious inside tank 6 instead of tank 4, which had not been declared gas-free before he entered the tank. The crew attempted CPR and requested emergency medical assistance from shore. Chau was pronounced deceased at a local hospital, dying from exposure to volatile hydrocarbons. Dung later admitted to the false information in two written statements provided to investigators. He further told investigators that he showed the modified mask to the vessel’s master, Nguyen Duc Nghi. He contended the master threw the mask into the sea, and told the crew to lie about it to the police so that Chau’s family would get the insurance money. The pump master, was sentenced to three months and two weeks in jail after his guilty plea. The cases against the master and chief officer for their role in the death were still pending in the Singapore courts.
